GlobalWorks 2020 – Five Required Forms for Confirmed Participants

Please submit your completed hardcopy forms to the GlobalWorks office in 110 Gerlinger Hall by 5 pm Monday, April 27, 2020.

1. Release and Assumption of Risk
Complete this form and submit hardcopy form to the GlobalWorks Office (110 Gerlinger Hall)

2. Student Behavior Agreement
Complete this form and submit hardcopy form to the GlobalWorks Office (110 Gerlinger Hall)

3. Internship Credit Agreement and Registration Form
Complete this form and submit hardcopy form to the GlobalWorks Office (110 Gerlinger Hall).  Yifang Zhang can act as the internship faculty supervisor for up to 8 INTL Internship Credits (3 credits for Pre-Med Shadowing programs, 6 for the Milan Program). If you would like her to supervise your credits and sign this form, please email her directly. If you would rather have the credits come from your home department, this form will need to be signed by a faculty member willing to oversee your credits.  Departmental faculty members or staff can contact Yifang with any questions.

4. Internship Health Evaluation Form
Make an appointment at the UO Health Center or with your family medical practitioner and have them sign this form.  Please follow the directions carefully as there are several parts to the document. If you are planning to use the UO Health Center, we recommend you make an appointment right away as appointments get booked up very quickly during spring term.

5. Travel Clinic Certificate

(only for internships in Asia, Central America, South America, and Africa)

This is required for internships in the following locations: Argentina, Mainland China, Colombia, Costa Rica, Ecuador, Hong Kong, India, Japan, Korea, Saudi Arabia, Senegal, Singapore, Thailand, and Vietnam. While students participating in internships in Australia, Canada, or Western Europe are not required to complete this form, we recommend you make an appointment with the Travel Clinic if you are not updated on routine immunizations (MMR, DTP, tetanus, etc.) or have any particular health concerns.  You can look up the health information about specific countries on the CDC Traveler website.
